Patients who saw Dr. Perryman when she was with Primary Pediatric Medical Association in Seguin, Texas, may send requests for medical records or shot records to the following address:

All request must be in writing. Please give us the name of the child, the child's date of birth, what you are requesting (complete medical record or shot record only), along with the requestor's name and relationship to the child. Also include either an address or fax number to return the copies to (records greater than 5 pages require a mailing address, we will not fax records consisting of more than 5 pages).

There is a $25 charge for each copy of a medical record.

There is no charge for the first copy of a shot record.

There is a $5 charge for each subsequent copy of a shot record.

Records from Primary Pediatric Medical Association are stored off-site. We attempt to fill all requests within 48-72 hours, however it may take as long as 15 days prior to request being filled. Requests requiring payment received without payment will be delayed until payment is received.

General questions regarding the contents of the Medical Record can not be provided over the phone as staff does not have access to this information.
