Fax request for Medical Records will no longer be accepted after August 29, 2014, as there will be no fax machine available for receipt.

Due to changes imposed at the national level (i.e. the Affordable Care Act) and changes by local employers in regards to insurance coverage for their employees, Dr. Perryman  had to make the very difficult decision to leave private practice.  Teri Perryman, MD closed her pediatric medical office in Kerrville on August 29, 2014.

Current and former patients may request medical records by clicking on the button below.  Both an on-line request form and mailing instructions are included on the Medical Record Policies page.

Medical Records must be maintained for seven (7) years after the last visit or until the child attains age twenty-one (21) whichever is longer.

Medical Records are being maintained for this time period by Dr. Perryman and her husband and will continue to be available per Texas Medical Board rules and regulations in effect at the time of the practice closing.

Dr. Perryman's office phone number will still be active after August 29, 2014 in order to leave messages related to medical record request and insurance claims/billing issues.
